Replacing the Home Care Package (HCP) and the Short Term Restorative Care Package, Support at Home (SaH) is the reformed home care scheme aimed at helping people who want to stay independent within their own home.
Launched on the 1st of November 2025, it offers 8 levels of funding, makes funding accessible to more Australian and provides quicker access to restorative care and end of life support.

Who Is Eligible?
Your eligibility and funding level are worked out through an assessment under the Single Assessment System. An assessor uses the Integrated Assessment Tool (IAT) to understand your needs. Once approved, you'll receive a priority rating (High, Medium or Standard), a funding budget and a care plan listing the services you can access.
Aged 65 years or older (50+ for First Nations people)
Australian Residents living at home and needing support to stay independent
Have been assessed by the Aged Care Assessment Team (ACAT)
